Registration Requirements

Proof of Residency

Residency must be established every year for all CMSD scholars. Students living with parents or guardian(s) must provide the district with at least two acceptable proofs of residency.

Please note the following apartments are not CMSD addresses:
Higgins McLaurin Arms Apartments
(905 Sasse)

Farmview Estate Apartments
(1015 Sasse)

Sunflower Lane Apartments
(2106 Center Street)

Campbell Place
(2586 Simmons Road)

Country Club Apartments
(113 Crestline Drive)

Parent(s) or legal guardian(s) of a student seeking to provide residency verification must have at least two of the items below as verification of their address.

NO post office box as an address will be accepted.

Acceptable Proofs of Residency:

The two (2) proofs of residency MUST BE one of the following  provided to CMSD by Parent, Guardian, or Other Adult:

  • Mortgage Documents or property deeds
  • Current Apartment or home lease
  • Current Utility bills ((dated within 60 days) showing service location within the Clarksdale Municipal School District. Accepted utility bills include; water bills, electricity bills, gas bills, and/or landline phone bills))
  • Driver’s License
  • Voter precinct identification
  • Automobile registration
  • Certified copy of a filed petition for guardianship if pending and final decree when granted
  • Any other documentation that will objectively and unequivocally establish that the parent or guardian resides within the school district; and, in the case of a student living with a legal guardian who is a bona fide resident of the school district.
    • Federal/State Documentation such as Social Security, Child Support, Medicaid, SNAP, TANF, etc. 

 

Requirements for  All incoming Pre-K Students

  1. Official Birth Certificate (no copies)
  2.  121 Form (Shot Record)
  3. Two (2) Proofs of Residency (bills must be within 60 days of the date you register)
  4. Physician Records (Dental and Vision)-complete assessment provided by your child's physician or doctor's office that includes dental and vision screening


Requirements for Returning Students (K-12):
Students who attended schools in CMSD during the 2023-2024 school year

  1. Official Birth Certificate (no copies)
  2.  121 Form (Shot Record)
  3. Two (2) Proofs of Residency (bills must be within 60 days of the date you register)


Requirements for New Students (K-12):
All NEW students grades K-12 who did not attend schools in CMSD during the 2023-2024 school year.

  1. Official Birth Certificate (no copies)
  2.  121 Form (Shot Record)
  3. Two (2) Proofs of Residency (bills must be within 60 days of the date you register)
  4. Report card from the last school attended
